This template uses Driscoll’s model (adapted from Driscoll 2007) but you might prefer to use other models you are familiar with. This is the same content as the form on nursing and midwifery ePortfolio. The Flying Start NHS website has lots of support and guidance around developing skills in reflection.

Structured reflective template for doctors undertaking a low volume of NHS GP clinical work

The Turas Professional Portfolio is recommended to help you record your reflections and can be used to develop evidence and record any supervision discussions. The Portfolio is available for or all Nurses, Midwives, Allied Health Professionals and Support Workers in Scotland including the NHS, social care, voluntary and independent sectors.

This reflective template has been designed to use with SOAR (Scottish Online Appraisal Resource). On completion, please upload this to SOAR for submitting to your appraisal to share with your appraiser.
